Older people have higher rates of chronic illness and are more likely to be taking multiple medications. Polypharmacy increases the risk of adverse drug events such as falls, 1 confusion and functional decline.2 Changes in physiology and social and physical circumstances contribute to the risk of adverse drug events. Older people are more likely to experience poor vision, hearing and memory loss and have altered metabolic rates, such as declining renal function. Adverse reactions may go undetected because symptoms may mimic problems associated with older age such as forgetfulness, weakness or tremor. Adverse reactions may also be misinterpreted as a medical condition and lead to the prescription of additional drugs.

Signs and Symptoms Fungal keratitis shows no predilection for age, gender or race.1 Often, patients have incurred corneal trauma, usually from organic vegetative matter. Other significant risk factors include ocular or systemic immunosuppression, either by disease or by medical immunosuppressive therapy such as for organ transplantation ; or topical steroid therapy, contact lens wear, and chronic antibiotic use.1-4 Fungal infections are also more common in agricultural and tropical environments.3 Finally, any breakdown of the corneal epithelial barrier increases the threat of invasive fungal disease. Fungal infections of the cornea often initially produce a feathery, branching pattern. Satellite lesions are not uncommon. The cornea takes on a dull, gray appearance, with possible heaping of epithelium and a dry, rough texture. In most cases, this characteristic appearance disappears over time, and fungal keratitis begins to resemble advanced bacterial keratitis. Misdiagnosis at this point is very possible, and there are no easy ways to clinically differentiate the conditions at this point. Typically, a severe anterior uveitis and plasmoid aqueous with hypopyon appears. There will be painful vision loss. Pathophysiology There are two types of fungi: molds and yeasts. Molds filamentary fungi ; are further subdivided into septate the most common causes of fungal keratitis ; and non-septate organisms. They produce feathery colonies that join together to produce hyphae. Yeasts, however, form pseudohyphae. The non-septate filamentary fungi are responsible for orbital disease but rarely infect the cornea. Of all possible fungal infections of the cornea, the vast majority are caused by Fusarium, Aspergillus both septate filamentary and pseudoephedrine.
